Using in-network providers is an easy way to save money on your out-of-pocket costs and receive the greatest value from your health insurance plan. CoOportunity Health plans are "open access" plans. This means you can schedule an appointment with any provider without a referral from a primary care provider.
The directories are organized by product (see gray toolbar above to select your product). After choosing your product, you can search for a provider based on location and specialty.
Although we update our directories on an ongoing basis, it is a good habit to ask if a provider is in-network before receiving care.
Our directories include provider address, phone number and specialty. If you would like more information about a provider's professional qualifications, including where they completed medical school and residency training, please call Midlands Choice Customer Services at 1.800.605.8259, option 5.

Non-emergency hospital services are coordinated by your treating physician. In an emergency, dial 9-1-1 or go to the nearest emergency facility.
