Coverage
This item may be covered at the durable medical equipment benefit stated in your member contract when your provider has ordered it.
Covered if it is a part of a treatment plan, such as for deep vein thrombosis or cellulitis. The set-up and delivery includes both the pump and pad. This is billed as one charge to the plan. A replacement pad and pump would be covered.
