Your Personal Information

“Personal Information” refers to specific data elements that can be used to identify or contact you as an individual. Personal Information includes, for example, your name, home address, phone number, birth date, gender, email address or other information that is unique to you. CoOportunity Health collects Personal Information on the Site only when you supply it, such as when you send an e-mail or sign up for or purchase a program, product or service through the Site, or when you visit the Site.

CoOportunity Health collects information about visitors to the Site from “cookies” and the Internet Protocol (“IP”) addresses of the computers used by visitors to access the Site. A cookie is a small text file created when a person uses a web browser to visit a website and is stored in the computer used to access the website. Most commercial websites use cookies or IP addresses to help visitors navigate the website and to help improve their site based on user behavior. If you do not want to have cookies stored on your computer, you can set your web browser to not accept them, or you can delete them manually.

CoOportunity Health maintains a variety of physical, electronic, and procedural safeguards designed to protect Personal Information and to prevent unauthorized access to Personal Information under our control. We require our vendors to do the same when they perform functions for us that involve Personal Information.

CoOportunity Health cannot and does not guarantee the confidentiality or security of Personal Information you send to us via the Internet or by email.   CoOportunity Health also cannot and does not guarantee that Personal Information may be kept confidential and secure from persons seeking to intentionally access Personal Information without permission and beyond our control.

How CoOportunity Health Uses and Shares Personal Information

We may use and share your Personal Information for the following purposes consistent with applicable laws:

  1.  To allow you to participate in a program you have enrolled in or to provide a product or service you have asked for or purchased, we may use your Personal Information and share it with our vendors and others to the extent necessary to make available the program or provide the product or service.
  2.  We may use and share Personal Information to the extent necessary to comply with a court or administrative order, law, rule, regulation, or similar requirement we believe in good faith legally requires us to use or share the Personal Information.
  3.  We may use and share Personal Information to the extent necessary to maintain and improve the Site, to protect the Site and our rights and to avoid or minimize our liability. For example, we may use Personal Information to identify a Site visitor who is using the Site for an illegal purpose and share Personal Information with law enforcement or our attorneys to consider taking legal action.

Notice Regarding Children under Age 13

The Site is not intended for use by persons under age 13.  If you are under age 13, you should not provide any Personal Information (for example, your name, address or email address) to us using the Site.  We will not knowingly collect any Personal Information submitted by users of the Site who are under age 13.  If we learn that you are under age 13, we will not maintain, use or share any Personal Information about you.
