An open access tiered benefit plan that provides flexibility for your employees to manage their out-of-pocket costs based on three categories (or Tiers) of providers.

Includes coverage for the 10 essential health benefits. Your employees’ out-of-pocket costs are lowest with Tier 1 providers (member organizations of UI Health Alliance and additional facilities and clinicians in select Iowa counties). Tier 2 providers included additional providers that participate in the Midlands Choice Premier network. Your employees’ out-of-pocket costs are higher with Tier 2 providers.
