CoOportunity Choice UI Health Alliance is an open access tiered benefit plan that provides flexibility to manage your out-of-pocket costs based on three categories (or Tiers) of providers.
Qualified Health Plan includes coverage for the 10 essential health benefits. Your out-of-pocket costs are lowest with Tier 1 providers (member organizations of UI Health Alliance and additional facilities and clinicians in select Iowa counties). Tier 2 providers included additional providers that participate in the Midlands Choice Premier network. Your out-of-pocket costs are higher with Tier 2 providers. Tier 3 providers are out-of-network. Your out-of-pocket costs are highest with Tier 3 providers.

plans. The next period for Open Enrollment is Nov. 15, 2014 -
Feb. 15, 2015.
Special Enrollment Period
